Privacy and your information
This is a prototype for review, accessible to anyone with the link. Please use invented test details, not real personal or confidential organisational information.
What is saved
When you submit the quiz, this site saves your name, email address, organisation, role or job title, ten answers, follow-up preference, consent and completion time. It also records a submission reference and the campaign source, if supplied in the link.
Before submission, your answers are held temporarily in this browser tab so a refresh can restore them. Your contact fields are not saved to browser storage. Temporary answers are cleared after successful submission.
How information is used
Your answers create your on-screen report. Authorised staff can view completed submissions and export them as a CSV. This review version does not send emails, subscribe you to a newsletter or automatically upload information to Salesforce.
The optional follow-up checkbox records whether you would like a conversation with AusDN. Leaving it unchecked does not prevent you from receiving your report.
Who can access submissions
The site stores submissions in its hosted database. The staff view and export require sign-in and explicit staff authorisation. Participants cannot browse other participants’ submissions.
Before public launch
AusDN will need to approve the collection notice, hosting arrangements, data retention period and staff access. This prototype notice is not a replacement for AusDN’s approved privacy policy.
